Nature of Telehealth Services
Telehealth involves the use of electronic communications to enable healthcare providers at different locations to share individual patient medical data for the purpose of improving patient care. Providers may include primary care practitioners, specialists, and/or subspecialists. The information may be used for diagnosis, therapy, follow-up and/or education, and may include any of the following: patient medical records, medical images, live two-way audio and video, and output data from medical devices and sound and video files.
Benefits and Risks
The benefits of telehealth include improved access to medical care by enabling a patient to remain in their home while the provider obtains test results and consults with practitioners at distant/other sites. Potential risks include, but may not be limited to: information transmitted may not be sufficient (e.g., poor resolution of images) to allow for appropriate medical decision making by the provider; delays in medical evaluation and treatment could occur due to deficiencies or failures of the equipment; and, very rarely, security protocols could fail, causing a breach of privacy of personal medical information.
Electronic Communication
I understand and agree that electronic communication is not 100% secure. OptimaVida Health & Wellness LLC utilizes HIPAA-compliant video and messaging platforms to minimize risk; however, I acknowledge the inherent risks associated with virtual transmission. I consent to receive texts, emails, and platform-based notifications regarding my care, scheduling, and health maintenance.
Patient Acknowledgments
I acknowledge that I have the right to withhold or withdraw my consent to the use of telehealth in the course of my care at any time, without affecting my right to future care or treatment. I understand that my provider will determine, according to their professional judgment, whether the condition being treated is appropriate for telehealth and that a physical examination may be required if deemed necessary for patient safety.
No Emergency Services
I understand that OptimaVida Health & Wellness LLC does NOT provide emergency services. If I am experiencing a medical emergency, I will immediately call 911 or go to the nearest hospital emergency department. Telehealth is not a substitute for emergency cardiac, respiratory, or acute trauma care.
Voluntary Consent
I have read and understand the information provided above regarding telehealth. I have discussed it with my healthcare provider and I am satisfied that all my questions have been answered. I hereby give my voluntary informed consent and authorize OptimaVida Health & Wellness LLC to use telehealth in the course of my diagnosis and treatment.
