Payment & Subscription Policy

OptimaVida Health & Wellness is committed to providing clear information regarding our membership-based service model and payment obligations.

1. Membership Structure

OptimaVida Health & Wellness LLC offers membership-based services including, but not limited to monthly and prepaid plans.

Monthly memberships:

  • Automatically renew
  • Are non-prorated
  • Require a valid payment method

Prepaid plans:

  • Include 3, 6, or 12-month options
  • Are offered at discounted rates
  • Do not auto-renew
2. Charges & Billing
  • Payments are charged according to the selected plan
  • Patients authorize recurring charges where applicable
  • Patients are responsible for maintaining valid payment information
3. Additional Costs

Services not included in membership, including but not limited to:

  • Laboratory testing
  • Imaging
  • Medications
  • External services

are billed separately.

4. Non-Refundable Policy

All payments are non-refundable, including but not limited to:

  • Membership fees
  • Prepaid plans
  • Laboratory orders
  • Services rendered
5. Modifications

OptimaVida Health & Wellness LLC reserves the right to modify pricing, services, or included features at any time.
